Your MIB Consumer File
When you apply for life insurance, the carrier asks your permission to check MIB — a shared database of underwriting information that member insurance companies contribute to and consult during underwriting. It is not a credit bureau, and it does not decide whether you're approved.
Know what's in your file before you apply
A file exists only if you've applied for individually underwritten life, health, disability, or long-term care coverage with a member company in the last seven years. If something in it is inaccurate or incomplete, you have the right to correct it. If you've received an adverse underwriting decision that cites your MIB record, you're entitled to an additional free copy.

Life Insurance Policy Finder
You can ask insurers to search their records for a life insurance policy or annuity contract belonging to a deceased person, at no cost.
Run by the National Association of Insurance Commissioners. Submit one request, and participating companies search their records for a policy or annuity in the deceased person's name. If a match is found, the company responds directly to you, provided you are the named beneficiary or otherwise authorized to receive the information.
Responses typically arrive over weeks, not days. No response means no participating company found a match.
